Abstract

Malrotation is a disorder of intestinal rotation and fixation. Most children with malrotation present within the first year of life. The most common presentation is onset of bilious vomiting without any abdominal distension or signs in a previously normal infant. Midgut volvulus is a life-threatening complication of malrotation which may result in bowel gangrene of most of the intestine. Diagnosis can made reliably in most cases with upper gastrointestinal (UGI) contrast study and Doppler ultrasonography. Even in asymptomatic cases with unequivocal diagnosis of malrotation, surgical intervention and correction of the deformity is recommended. Malrotation associated with midgut volvulus is a surgical emergency and should be operated at the earliest. Complications after surgery are rare with adhesive intestinal obstruction being the most common.
