Abstract

This chapter focuses on medical records. This chapter is prepared with the objectives to introduce a modern and scientific medical record system in conformity with international standards, establish and maintain uniform and comprehensive medical records for all patients, provide standard guidelines for organizations, functions, operational policies and procedures in maintaining effective patient records, develop and effectively manage the medical record department, so as to assist in efficient patient care, medical education, research and other administrative activities, maintain confidentiality and protect legal interest of patients, employees and hospital, and collect and supply health information needed for finance, quality control and efficient management of health center.
