Multidrug resistance (MDR) is defined as resistance to at least one agent in three or more antimicrobial classes. Emergence and spread of MDR is almost an international hazard of epidemic proportions. Since most MDR infections involve urinary tract, it is a medical and social responsibility of urologists to prevent and identify infections early and to treat them effectively. As it is evident from the study of epidemiology and risk factors, there are certain risk factors common for most varieties of MDR infections. Prolonged stay in hospital, especially in intensive care unit (ICU), with patient on prolonged ventilatory support, presence of chronic indwelling catheter, immunocompromised with diabetes mellitus, malignancy, transplantation, trauma, and burns contribute to rapid development of MDR. Education of healthcare workers and close cooperation between urologists, nursing staff, ICU clinicians and infection control specialists will go a long way in effective management of this difficult problem.
