Abstract

Transseptal puncture is an essential step in performing percutaneous mitral valvotomy (PMV) through the antegrade route. The transseptal puncture technique was developed by Ross, Braunwald and Morrow at the National Heart Institute (now the National Heart, Lung, and Blood Institute), Bethesda in the late 1950s to allow left heart catheterization, principally for the evaluation of valvular heart disease. This chapter covers how to perform transseptal puncture, steps in transseptal puncture, landmarks for septal puncture, needle orientation, septal puncture without arterial stick, and probing the septum. Transseptal puncture is done usually under fluoroscopic guidance. There are certain fluoroscopic landmarks which are used to identify the location of the fossa ovalis where we usually puncture the septum. The pigtail catheter marks the location of the aortic valve.
