This chapter covers the patient profile, chief complaints (on the day of care), present medical history, description of chief complaints (cold spa), past medical history, present surgical history, past surgical history, family history, socioeconomic history, and personal history. Onset of illness, duration, treatment done before admission, treatment provided in the present hospital. Chief complaints (cold spa) include character, onset, location, duration, severity, pattern, associated factor.
