Abstract

The purpose of sclerotherapy is to produce endothelial damage that results in permanent endofibrosis and clinical obliteration of the vessel. This can be achieved by a substance called sclerosant. This chapter covers the general mechanism of action of sclerosants, detergents, osmotic solutions, chemical irritants, sequential injections of different sclerosing solutions, and ideal sclerosant. Sequential injections may be useful to enhance the efficacy of a milder sclerosing solution, either by increasing its potency or by the act of sequentially damaging endothelium. In addition, combining a solution with another may produce an additive effect on its potency. The ideal sclerosing solution should be painless on injection, free of all adverse effects, and would not produce allergic reactions and specific for damaged (varicose) veins.
