This chapter briefly reviews the modifications of tissue grafting techniques like suction blister grafting as a treatment option in vitiligo. The chapter explains donor site selection, choice of anesthesia, inducing blisters, troubleshooting in nonformation of blisters and vesicles, handling of grafts, preparation of recipient site, transfer of grafts to recipient site, and dressing in suction blister grafting. 3-layer dressing is advocated. The first layer being a gauze impregnated with Soframycin or paraffin, the second layer of moist gauze, and third layer of dry gauze. This helps in maintaining postoperative hydration and better graft survival. Excellent repigmentation, lesser learning curve, minimum chances of complication and being one of the most economical options, suction blister grafting still continues to be a excellent surgical modality for vitiligo surgeons, in the era of cellular grafting.
