Abstract

Small incision lenticule extraction (SMILE) is a relatively recent addition to the world of refractive surgery and emerging as a safe and successful alternative to femtosecond laser-assisted in situ keratomileusis (FS-LASIK) for the correction of refractive error. Comprehensive patient evaluation and preoperative counseling help achieve optimal outcomes while maximizing patient satisfaction. This chapter covers the clinical history, clinical examination, investigations, decision-making and case selection, and patient counseling and informed consent. Proper patient counseling is an important step in ensuring good results after any refractive surgery. The surgeon should assess the patient’s expectations before deciding for the surgery. Unrealistic expectations like an improvement beyond the BCVA or perfect vision after refractive surgery should be dealt with appropriately.
