Abstract

The chapter describes about the cleft lip and cleft palate. Face develops from the cranial neural crest cells. At 5th week of intrauterine life, frontonasal process divides into right and left parts. The junction of upper lip with skin is known as white line. Cleft lip occurs as a result of defect in fusion of processus globularis with maxillary process. Thus, in upper lip, the cleft is always on one side of midline and not in the midline. In lower lip, if mandibular processes fail to fuse in midline, it produces median cleft lip. It occurs very rarely. Cleft palate occurs due to failure of fusion of palatal processes of maxilla and occurs in midline. The cleft may involve lip, palate or both. The cleft lip may be unilateral or bilateral. The aim of surgical repair in cleft lip is to give cosmetically acceptable face so that there is no psychological problem for parents of the patient. Surgical repair of cleft palate aims to provide intact roof to the mouth and a mobile soft palate so that it reaches posterior pharyngeal wall on phonation and produces competent nasopharyngeal sphincter, and to give well-aligned teeth and no loss of hearing.
