Health assessment is a vital component of nursing process. It is the health assessment around which entire nursing care is directed. Nurses are employed in different health settings, seeking baseline information about client’s health status. A thorough health assessment involves a detailed health history and physical examination. A physical examination is a head-to-toe review of all body system that provides objective data. This chapter covers the physical examination, and guidelines for health history interview. Physical examination is the systematic, organized, superficial check of body systems to detect any abnormality or problems.
