Abstract

This chapter highlights the need of educating women with diabetes about significant role of contraception and various contraceptives options available to achieve a good prepregnancy glycemic control. In females with type 2 diabetes mellitus (T2DM), intrauterine devices (IUDs) are effective and safe to use in all females. Both copper IUD and levonorgestrel intrauterine system (LNG IUS) are safe, while risk of bleeding is lower with LNG IUS. Combined oral contraceptives can be given in females with T2DM, but should be avoided in those more than 40 years of age, hypertensive, having family history of cardiac events, migraine, or history of smoking. Third generation combined oral contraceptives (COCs) should be preferred because the risk of MI and thrombosis being low with them. Nonoral hormonal contraception and progestogen-only pills (POPs) have similar side effect profile as oral COCs, and can be used when compliance is an issue.
