The International Vitreomacular Traction Study (IVTS) Group4 developed optical coherence tomography (OCT) based, strictly anatomic definitions and a classification system for diseases of the vitreomacular interface, like vitreomacular adhesion (VMA), vitreomacular traction (VMT) and macular hole. The VMT syndrome can occur at any age and there is no racial predilection. It affects both sexes, although women show a slightly higher incidence because of premature vitreous liquefaction and an earlier onset of PVD favored by low estrogen postmenopausal state. OCT is an excellent noninvasive imaging tool to visualize vitreoretinal interface and is useful for the diagnosis and management of VMT syndromes. The VMT can be managed using pars plana vitrectomy, pneumatic vitreolysis, enzymatic vitreolysis, and observation. Pars plana vitrectomy (PPV) is the surgery of choice to relieve all the tractional forces at the vitreoretinal interface. A brief discussion on pneumatic vitreolysis has been discussed in this chapter.
