BOOK TITLE: Retina: Medical and Surgical Management
Cytomegalovirus (CMV) retinitis, acute retinal necrosis, syphilitic chorioretinitis, endogenous endophthalmitis are various types of infectious inflammation, which are highlighted in this chapter. CMV retinitis is the most common ocular manifestation, and the most common cause of visual loss in patients with acquired immunodeficiency syndrome. Treatment of CMV retinitis can be local or systemic, and split into two phases: induction and maintenance phase. Acute retinal necrosis is classically caused by herpes simplex virus (HSV) or herpes zoster virus (HZV), and may occasionally be caused by CMV or Epstein-Barr virus. With a wide range of antiviral agents currently becoming available, physicians are resorting to single agent therapy or varying combinations of intravenous, oral and intravitreal drugs for initial and long-term treatment of acute retinal necrosis. Endophthalmitis is an ocular emergency with devastating visual prognosis and multifactorial etiology. A high index of suspicion is required for timely diagnosis of Endogenous endophthalmitis. Lyme disease, toxoplasma retinochoroiditis, toxocara chorioretinitis, and tuberculosis are also covered in this chapter.
