The aim of this chapter is to focus on the surgery for proliferative diabetic retinopathy. Diabetic retinopathy is primarily a disease of retinal microvasculature, and is characterized by progressive retinal ischemia resulting in development of new blood vessels in the late phase [proliferative diabetic retinopathy (PDR)]. Robert Machemer performed his first vitrectomy in a patient with nonresolving VH secondary to PDR in 1970. Since then, surgical interventions for PDR have continued to evolve. Types of vitreoretinal adhesions, indications for surgical intervention in proliferative diabetic retinopathy, preoperative evaluation, surgical techniques, additional procedures, antivascular endothelial growth factor as an adjunct to vitrectomy, and surgical complications are covered in this chapter.
