The field of vitreoretinal surgery (VR surgery) has seen several advances in the past few decades. With improvement in instrumentation and technology, the indications for surgery have also expanded. Complications of VR surgery, both straightforward and complex, can be broadly classified into those involving the anterior segment and posterior segment. Other ways to classify include intraoperative, postoperative, or delayed complications, and whether instrument, technique, or iatrogenic related. With the appropriate preparation and management, VR surgery complications can be minimized and resolved. This chapter provides a brief overview of etiopathogenesis and treatment of complications of VR surgery. Cataract is the most frequent complication following vitrectomy procedure. Cataract can either develop secondary to the internal tamponades used or directly related to the surgical procedure.
