This chapter describes the newer innovations and techniques in vitreoretinal surgery. The last decade has seen major advances in the field of vitreoretinal surgery. There have been many technological breakthroughs, which have expanded the indications of vitreoretinal surgery. The efficiency, speed, and safety of posterior segment surgeries have markedly improved. Few recently introduced technologies, like intraoperative optical coherence tomography (iOCT) guided vitreoretinal surgery, have opened new frontiers for us that are yet to be explored. This chapter covers the small gauge vitrectomy, improved viewing and illumination, intraoperative surgical microscope with integrated optical coherence tomography and heads-up display, three-dimensional viewing in vitreoretinal surgery, robotic eye surgery, vitreous substitutes, and retinal implants. Optical coherence tomography is a rapid, noncontact, and noninvasive high-resolution optical biopsy of tissue microstructure, which has revolutionized our understanding of the pathology of various retinal disease and guides therapeutic decision-making. Seamless integration of this technology into ophthalmic surgery has now laid the foundation for a new paradigm in the surgical management of ophthalmic disease.
