Abstract

Today, liver transplantation is widely accepted as the preferred treatment for adults and children with end stage liver disease and for selected patients of acute liver failure. This has been made possible because of progress and continuing refinement in surgical technique, immunosuppression and perioperative care of these patients. The anesthetic management for this procedure is quite challenging and involves managing complex issues of very dynamic changes in the coagulation and circulatory parameters through various phases of the surgery. This chapter covers the liver transplant indications, objective assessment of liver disease severity, preoperative evaluation, anesthesia preparation and premedication, anesthetic technique, phases of transplantation, specific complications of transplant procedure, intraoperative transfusion of blood and blood products, primary nonfunction/initial poor functioning graft, and postoperative care.
