Abstract

Coronary artery disease (CAD) is more common in diabetics and is more extensive and diffuse. Relative risk of acute myocardial infarction (AMI) is 50% higher in diabetic men and 150% more in diabetic women. Sudden death is 50% higher in diabetic men and 300% higher in diabetic women indicating higher prevalence and mortality. Thus diabetes mellitus (DM) is an important risk factor for mortality in patients with ST-segment elevation myocardial infarction (STEMI). Diabetic patients often present with the atypical presentation in unstable angina and myocardial infarction. That atypical presentation of acute coronary syndrome (ACS), more frequent in diabetic patients, includes angina equivalents such as nausea, vomiting, diaphoresis, epigastric discomfort, dyspnea and an unexplained fatigue.
