Abstract

Diabetic cardiomyopathy (DCM) was first described 40 years ago, based on postmortem observations in four diabetic patients with heart failure but no coronary artery disease or other etiological conditions explaining heart failure. The authors observed left ventricular (LV) hypertrophy associated with myocardial fibrosis in those patients and introduced for the first time the concept of DCM. This chapter covers the experimental evidences for diabetic cardiomyopathy, epidemiological evidences for diabetic cardiomyopathy, structural myocardial abnormalities associated with diabetes, morphological and functional abnormalities, functional abnormalities, microvascular alteration, therapeutic perspectives, and cardiac resynchronization therapy.
