Endocrine emergencies are dealt by internists more than endocrinologists, so a thorough knowledge of handling these patients is mandatory for all emergency physicians and internists. Prevention of adrenal crisis is done by patient education and appropriate dose adjustment of glucocorticoids under stressful situations like minor and major surgical procedures, infection, fever etc. Pituitary apoplexy refers to a clinical syndrome, characterized by sudden onset of headache, vomiting, visual impairment and decreased consciousness caused by hemorrhage and/or infarction of the pituitary gland. Liasion with endocrinologist and neurosurgery is essential component for successful management pituitary apoplexy. An undiagnosed or improperly treated pheochromocytoma can cause severe morbidity or even mortality. Hypertensive crisis in pheochromocytoma is a hypertensive emergency. Thyrotoxic crisis is an acute, life threatening, hypermetabolic state induced by iatrogenic or spontaneous release of thyroid hormones in a patient with uncontrolled thyrotoxicosis. Myxedema coma, diabetic emergencies and diabetic ketoacidosis, hypoglycemia, acute hypercalcemia, and hypocalcemia are the other endocrine emergencies which are also discussed briefly in this chapter.
