This chapter describes guidelines for fluid resuscitation in trauma patients. Trauma is a major cause of morbidity and mortality all over the world. Trauma induced coagulopathy (TIC) is an endogenous hypocoagulable state that occurs in the setting of tissue hypoperfusion and is primarily mediated by activation of protein C. Severe injury accompanied by tissue hypoperfusion leads to increased endothelial and circulating thrombomodulin, which subsequently binds thrombin. This chapter covers the pathophysiology, recommendations, damage control resuscitation, assessment of the patient on arrival, fluid therapy, blood products, special situations, turning off massive transfusion, adjuncts to fluid therapy, and future trends.
