Abstract

This chapter focuses on Traumatic Postpartum Hemorrhage—The Forgotten Entity. Population-based surveys have reported a wide variation in the prevalence of PPH form 5% to 17.5%. The challenge is that the reduction in mortality cannot be achieved just by enhancing skills and technology but will require strategies. The various causes of PPH are atonic, traumatic, retained placental bits, coagulation disorder, mixed variety and placenta accreta. Traumatic PPH contributes to 10–20% of all PPH. Various types of trauma may be laceration of cervix, vagina and perineum (vulvar hematoma) and paraurethral tear. Others may be due to rupture of uterus, extension of cesarean section (CS) wound, broad ligament hematoma and uterine inversion. Out of all traumas vulvar hematoma may be forgotten at times. Proper perineal examination can detect vulvar hematoma.
