The aim of this chapter is to focus on the intrapartum management of diabetic pregnancy. Diabetes is a common medical condition complicating 6–9% of pregnancies. Pregnancy with diabetes is a high-risk pregnancy because of the associated fetomaternal complications in antepartum, intrapartum, and postpartum periods. Complications in the intrapartum period include maternal complications such as preterm labor, prolonged labor, genital trauma, postpartum hemorrhage (PPH), sepsis and fetal complications such as prematurity, shoulder dystocia, brachial plexus injury, stillbirth, respiratory distress, neonatal hypoglycemia, hypocalcemia, hyperbilirubinemia, and polycythemia. This chapter covers the timing and mode of delivery, blood glucose control during labor and birth, obstetric management, and immediate postpartum care.
