Abstract

Infertility is defined as the inability to conceive after 1 year of regular unprotected intercourse. The incidence of infertility is on the rise. Diabetes mellitus is a metabolic disorder. It mainly involves abnormality of carbohydrate metabolism. The disease process is multipronged and has widespread ramifications. It involves vital organs such as eyes, nerves, kidneys, liver, blood vessels, and reproductive tissues as well. Type 1 diabetics are genetically predisposed and it gets activated by the environmental factors. Hyperglycemia and diabetes are implicated as causative and contributory factors in the development of subfertility and infertility in both men and women. This chapter covers the risk factors common for infertility and hyperglycemia, diagnosis, management, and oral hypoglycemic agents. Management of diabetes involves a holistic approach which includes lifestyle management and pharmacotherapy. The two most important drugs implicated in the treatment of PCOS are metformin and inositols. Metformin and inositols improve oocyte quality in infertile PCO patients and convert anovulatory cycles to ovulatory.
