Abstract

The aim of this chapter is to focus on the assessment of ovarian reserve. Ovarian reserve is the functional potential of both the ovaries at a given age and reflects the quantity and quality of remaining primordial follicles. The true ovarian reserve or the resting pool of primordial follicles is different from the functional or the growing pool, which can be recruited by exogenous hormones to preovulatory stage in assisted reproduction. Both represent different stages of follicular development and decline with age. Ovarian reserve, in turn, imitates the fertility potential of a female. There are several important indications for assessment of ovarian reserve such as counseling of patients regarding prognosis of achieving pregnancy, determining the starting dose for ovarian stimulation before first treatment cycle, and predicting hyper or poor response to ovarian stimulation. The ideal ovarian reserve tests (ORT) should be reproducible, convenient, economical and demonstrate least inter or intra-cycle variability. The test should have validity, i.e. good sensitivity and specificity.
