Hysteroscopy

by Rita Mhaskar

Exploration of human body cavities had intrigued mankind for centuries. Hysteroscopy was one of the very early attempts for direct visualization of the uterine cavity through an endoscope. Ironically, advances in hysteroscopy had to wait for technical innovation in other endoscopic fields before the technique became feasible in ordinary clinical practice. Various problems impeding the progress in the field of hysteroscopy included means of distending the uterine cavity, friable nature of the uterine mucosa and frequent need for dilatation of cervical canal needing anesthesia. Since early 1980’s, revival of hysteroscopy has opened up new diagnostic vistas for evaluation of the cervical canal and uterine cavity obviating the limitation of dilatation and curettage. Surgical interventions carried out through the hysteroscope have demonstrated superior results to traditional surgery of the uterus. Management of abnormal uterine bleeding by means of hysterectomy can now be largely replaced by endometrial resection/ablation through the hysteroscope which preserves the integrity of the urogenital tract. Recent innovation in the form of office hysteroscopy has now further revolutionized the field. A comprehensive examination of the cervical canal and uterine cavity can now be performed in an office setting without using any anesthetic or dilating the cervical canal. Hysteroscopy is fast becoming a gold standard in the evaluation of uterine cavity and cervical canal. Training in Gynecology is incomplete without an ability to at least perform a diagnostic hysteroscopy. This book is a comprehensive effort in this direction, targeting consultants and residents in Gynecology. It is a step by step effort in an easy language towards training in Hysteroscopy both diagnostic and operative.
