This chapter divided into three parts i.e. hallux rigidus, hallux varus and disorders of sesamoids. Hallux rigidus describes a painful condition of the metatarsophalangeal (MTP) joint of the great toe associated with restricted motion, mainly dorsiflexion and periarticular new bone formation. Conservative treatment and surgical treatment are the two treatment options for hallux rigidus. Hallux varus is characterized by adduction of the great toe and medial subluxation of the first MTP joint. Congenital Varieties and Acquired Varieties are two treatment options of hallux varus. Sesamoids, embedded in the tendons of flexor hallucis bravis, play important role in weight absorption, protection and providing mechanical advantage to muscles around the first ray. Sesamoid dysfunction can occur due to trauma, arthritis, infections osteochodritis and sesamoditis. Shaving of sesamoid, sesamoidectomy and internal fixation/bone grafting are the surgical treatment options for sesamoids.
