Abstract

The main focus of this chapter is on neonatal septic arthritis, neonatal osteomyelitis and management of sequelae of childhood bone and joint sepsis. Septic arthritis is an acute joint infection caused by pyogenic bacteria. Septic arthritis is more common than osteomyelitis in neonates. It is very common in preterm, low-birth-weight neonate and child with pre-existing sepsis. Osteomyelitis is inflammation of bone and assumed to be infective (bacterial) even if there is no growth in culture. Normally, osteomyelitis is not common in neonatal period but shows a clear increased incidence during childhood. The incidence of septic arthritis/osteomyelitis in India is approximately 1 in 1,500 live births. In osteomyelitis, the bacteria get inoculated in the metaphysis initially due to local vascular anatomy and less macrophage concentration. Diagnostic delay and late referral, over-reliance on antibiotics, lack of use of advanced imaging, newborn with comorbidities and failure to do complete debridement are the risk factors responsible for developing sequelae.
