Abstract

This chapter describes the Perthes disease. Perthes disease refers to aseptic noninflammatory self-limiting idiopathic avascular necrosis of capital femoral epiphysis in a child. A child with Perthes disease usually presents with pain in the hip with a limp. Perthes disease is usually diagnosed with a plain radiograph. The avascular necrosis may involve either part of the epiphysis or the entire epiphysis. The primary aim of the treatment of the Perthes disease is to prevent secondary osteoarthritis of the hip by preventing the femoral head from getting deformed. The age of onset of symptoms, the extent of epiphyseal involvement, the stage of the disease and the presence of epiphyseal extrusion are the factors to consider before planning treatment for Perthes diseases. The containment can be achieved by abducting and internally rotating the femur or abducting and flexing the femur. Nonoperative containment and operative containment are the different types of containment.
