Abstract

This chapter covers appendageal tumors and presents color images of follicular differentiation, sebaceous differentiation, apocrine differentiation and eccrine differentiation. Transverse sections of biopsies are better for detecting pathological changes in follicular microanatomy. Sebaceous adenoma situated deep in the dermis with cystic change may be a marker for Muir Torre syndrome. Multivesicular bubbly cytoplasm differentiates sebaceous carcinoma from other clear cell tumors. Sebaceous and apocrine glands in nevus sebaceous become prominent only after adolescence. Desmoplastic trichilemmomas can be differentiated from squamous cell carcinoma (SCC) and BCC by PAS stain which shows an intact peritumoral basement membrane. In older lesions of pilomatrixoma, the basaloid cells are completely replaced by ghost cells and dystrophic calcification is seen. Mitotic figures are uncommon in microcystic adnexal carcinoma.
