Abstract

This chapter focuses on granulomatous tissue reaction and provided images of tuberculoid granulomas, leprosy, suppurative granulomas, sarcoidosis, palisading granuloma, foreign body granuloma, and miscellaneous conditions with granulomas. The presence of birefringent crystals on polarization in the granulomas can be seen in sarcoidosis in addition to foreign body granulomas. Early lesions of granuloma annulare may not show palisading granulomas. There is macrophage infiltration between collagen bundles and increased mucin. Subcutaneous palisaded granulomas with central fibrinoid necrosis lacking mucin are characteristic of rheumatoid nodules. Fontana Masson stain is useful in identifying Cryptococcus in patients of AIDS infected with low virulence. Cryptococcus stains with little capsular material. It stains the organism black. The yeast forms of histoplasma do not stain with Gram stain. Asteroid bodies in sporotrichosis are extracellular as compared to sarcoidosis. The use of an immunoperoxidase technique assists in making a specific diagnosis of histoplasmosis on paraffin-embedded material.
