This chapter covers discussion over pattern of hair shafts with the help of clinical images. Upright regrowing hairs (URH) are also called short regrowing hairs and found in the normal scalp, in the remitting phase of alopecia areata (AA) and telogen effluvium (TE). Vellus hair is defined as hypopigmented, nonmedullated hairs less than 30 micrometer thick and less than 2–3 mm long; up to about 10% of normal scalp hair are vellus hairs. Circle hairs are short vellus hairs and are also known as pigtail hairs. High numbers of coiled hairs in a field of trichoscopy are almost diagnostic of AA. Broken hairs are mostly found in TTM, tinea capitis, alopecia areata and after hair styling procedures. Trichoptilosis hairs have split ends and characteristically they are irregular and uneven. Tapering hairs include exclamation mark hairs and coudability hairs. They are highly suggestive of AA where they are found at the periphery of the patches. Flame hairs, coiled hairs, comma hairs, corkscrew hairs, tufted hairs, hair diameter diversity, dark lines, zigzag hairs/Z hairs, medulla, pohl-pinkus constriction, etc. are also covered in this chapter.
