This chapter aims to cover pattern of scaly scalp with the help of clinical images. Scales can be seen around the follicles, around the hair shafts and in the interfollicular space. Scales could be a useful clue to reach a diagnosis. Perifollicular scales may be found in lichen planopilaris (LPP), frontal fibrosing alopecia (FFA), diascoid lupus erythematosus (DLE), ichthyosis vulgaris, folliculitis decalvans etc. Interfollicular scales are seen in psoriasis, seborrhoeic dermatitis, ichthyosis vulgaris, tinea capitis, DLE etc. Caution should be exercised while differentiating between hair casts and nits. Nits have an oval shape and do not surround the hair shaft. Use of dry dermoscopy is recommended to identify scales.
