Abstract

Discogenic pain refers to back pain arising from the disc itself. Discogenic low back pain (DLBP) is the most common disease of chronic low back pain, accounting for 39% of its incidence. The etiology of DLBP has not been definitively established, but it probably results from a compression injury causing an end-plate fracture. This injury triggers inflammatory degradation of the nucleus pulposus and eventually of the annulus fibrosus (AF) in the form of annular fissures. A detailed history and physical examination can be helpful in determining the etiology of low back pain. Diagnostic evaluation by magnetic resonance imaging (MRI) plays an important, but not exclusive, role in the diagnosis of internal disc disruption (IDD). Discography, a diagnostic procedure designed to ascertain whether a disc is intrinsically painful, is the single most important test for diagnosing IDD. Numerous nonsurgical percutaneous intradiscal treatments for IDD have been developed with varying degrees of success. These procedures are used to treat intradiscal (contained) versus extradiscal (extruded) herniations.
