Abstract

The choice of incision in the neck is critical in the preoperative planning of surgery for tumors of the head and neck. The ideal incision allows adequate exposure of all the lymph node levels that need to be dissected, as well as easy access to and ample exposure of the primary tumor. Today, there are several practical incisions that the head and neck surgeon can choose, depending on location of the primary tumor, stage of the neck, type of cancer, and previous performance of an ill-planned excisional or incisional biopsy of a neck node. This chapter describes those incisions that found practical and have used many times over the years to perform the entire gamut of neck dissections. These include modified Schobinger incision, modified Mcfee incision, unilateral apron-like incision, upper transverse incision, midneck transverse incision, low transverse incision, unilateral hockey stick incision, and bilateral hockey stick incision or long apron-like incision.
