Abstract

Modified radical neck dissection consists of the removal of the lymph node bearing tissues of one side of the neck (levels I–V) preserving the spinal accessory nerve. Like in the radical neck dissection, the internal jugular vein (IJV) and the sternocleidomastoid muscle (SCMM) are included in the resected specimen. Moderate to severe electromyographic abnormalities and temporary dysfunction of the trapezius have been observed in patients that have undergone this operation. Currently, the main role for this type of neck dissection is in the surgical treatment of the neck in selected patients with clinically obvious lymph node metastases, whenever the spinal accessory nerve is not directly involved by tumor, regardless of the number, size and location of the involved lymph nodes. Surgical technique of modified radical neck dissection has been described broadly in this chapter with the help of well-illustrative figures.
