It is an honor and great privilege to write this foreword. Seth Thaller and Mimis Cohen are both renowned educators, forward thinkers and internationally recognized leaders in plastic surgery.
Obesity is an epidemic; however, bariatric surgery has been proven to be an effective and safe option for patients afflicted with massive obesity, a condition that is resistant to traditional diet and exercise regimens. Long-term studies show that the procedures used cause significant long-term loss of weight, recovery from diabetes, improvement in cardiovascular risk factors, and a reduction in mortality.
In the past, reconstructive and cosmetic surgeons have developed new, innovative techniques in craniofacial, micro- and endoscopic surgery in order to meet the needs of patients suffering from burns, cancer and trauma. In a similar way, plastic and reconstructive surgeons have once again risen to the challenge of rapidly developing new and safe techniques to help post-bariatric patients to deal with redundant skin and fat.
The editors have assembled a group of knowledgeable experts who have contributed to this comprehensive and practical text covering all aspects of cosmetic surgery after massive weight loss. The book features contributions from numerous residents and young colleagues, as well as very experienced authors who are leaders in their fields and who have already published extensively on this challenging topic.
Among the interesting topics covered, I was particularly fascinated by the by first few chapters, which describe the surgical options, psychological evaluations, and medical management of post-bariatric patients. Subsequent chapters deal with cosmetic surgical options that improve various anatomical areas after massive weight loss. The final chapters report on new, unique approaches to tissue transfer for cancer reconstruction, techniques to avoid unfavorable results, and important medicolegal issues related specifically to cosmetic surgery for the massive weight loss patient.
Congratulations should go to the editors and the very distinguished group of authors responsible for developing this landmark contribution to plastic surgery. This book should become a key focal point for cosmetic surgeons specializing in body contouring surgery for massive weight loss patients.

Due to the almost epidemic proportion of the population afflicted with morbid obesity and associated morbidity, there has been an ever expanding number of patients undergoing bariatric surgical procedures. Postbariatric body contouring surgery is centered on enhancing the overall quality of life following weight loss surgery. This focuses on the overall loss of skin tone, resultant sagging fat and skin, and concomitant functional deficiencies associated with these clinical issues. At the completion of the plastic surgical rehabilitation, the patient should enter a final stage where they are able to obtain a more natural look. Prior to embarking on this surgical endeavor, the plastic surgeon must ensure that the patient is an appropriate candidate for surgical intervention. Key considerations are that the patient's weight is stabilized, they have developed a new, healthy and fit lifestyle, they have satisfactory motivation, are medically stable, and possess reasonable goals and expectations. However, all surgical procedures and sequences must be individualized and may even require serial operative approaches. The priority must always remain patient safety.
This book has solicited experts and innovators in their field to impart their experiences with these challenging patients. The authors have shared their extensive knowledge and have covered every aspect of the comprehensive management of post-bariatric patients, from the actual options for weight loss surgery, to the psychological needs of the individual patient. The authors then describe how to approach the patient and develop an overall treatment plan. With these goals in mind, this book will serve to introduce the plastic surgeon early on in their career to how best to approach and successfully manage these patients, and importantly, avoid complications. In addition, the more seasoned plastic surgeon will be able to employ this book to review, update, and enhance their skills prior to embarking on operative treatment.
