5Preface
Unfortunately there are many people who still have no access to the basic eye surgery which could make such a difference to their lives. At present about 60% of patients requiring eye surgery in Asia receive it, whereas in some parts of Africa this figure is as low as 20%.
The purpose of Eye Surgery in Hot Climates is to describe simple, cost-effective procedures appropriate for countries with a low income. Eye surgery technology continues to advance in developed countries; however, much of this technology does not benefit patients in developing countries due to the expense. This book gives advice on those techniques which are within reach of health care professionals working in resource poor environments.
One of the main changes that has taken place since publication of the last edition is the reduction in cost of good quality intraocular lenses, the use of which has become standard practice worldwide. Moreover, global initiatives in support of blindness prevention have been scaled up enormously in recent years. Surgical training lies at the core of both of these developments. This book has been written with the goal of promoting these skills.
We hope that Eye Surgery in Hot Climates, 4th Edition, will prove invaluable to those in training and continue to provide practical advice for health care providers in developing countries.

6Dedication
We dedicate this book to Dr. Nick Metcalfe. He built up the Eye Hospital in Nkhoma, Malawi, to be the most productive department in Southern Africa, and a flagship for training and surgical excellence. If he had not sadly been stricken with a disabling disease four years ago, he would have been a co-author of this book.
