The rapidly growing population due to (a) Low fertility rate (b) High birth rate against low death rate (c) Increase Teenage pregnancy (d) Poverty (e) Poor RCH care, had been a major headache for health planners and administrations, in India since independence.
Inspite of National Planning Program, maternal and child health (MCH) care, the child survival and safe motherhood (CSSM) program (1992), the desired impact on the population growth and health and development of women and children in the country could not be achieved and the need for a new approach to the problem was felt.
Following International Conference (1994) on population and development (ICPD) held in Cairo, India also agreed to implement reproductive and child health (RCH) program in October, 1997. The RCH program is to provide an integrated package of services, which will include the following:
  1. Services for mothers during pregnancy, childbirth and postnatal period including safe abortion services when required.
  2. Services for children including newborn care immunization, vitamin in A prophylaxis, oral rehydration therapy (ORT) for diarrhea, management of acute respiratory infections (ARI), anemia control, etc.
  3. Services for eligible couples through promotion of use of contraceptive methods as well as infertility services when required.
  4. Prevention and management of reproductive tract infections (RTIs), and adolescent health services including counseling on family life and reproductive health.6
For rendering the above services the new approach under the RCH program places special emphasis on client-oriented, need-based, high quality, integrated services to the beneficiaries. There has been a major shift/change in the approach from the past and some important ones among these are :
  1. Target-free program based on community needs.
  2. Participatory planning.
  3. Greater emphasis on quality of care and client satisfaction.
Main objects of this booklet are divided into different categories which include:
  1. Fact on maternal and neonatal mortality and morbidity in India.
  2. The state on maternal health.
  3. Health system on India.
  4. Policies and programs.
  5. Steps to reduce maternal mortality and morbidity at domiciliary, Phc, rural hospital and district hospital.
  6. Board guidelines for referral during pregnancy and child.
  7. Fact on MTP and family planning.
And finally for better understanding on RCH. It is divided into three areas.
  1. SAFE MOTHERHOOD CRISES — is highlighting the various crises during pre-reproductive, reproductive and post-reproductive period.
  2. SAFE MOTHERHOOD CARE — is basically for prevention of pediatric and adolescent diseases, complication of pregnancy and childbirth, family planning program and post-reproductive period.
  3. SAFE MOTHERHOOD CURE — is meant for prompt and safe management of any problem arises during pregnancy and childbirth, family planning procedure and post-reproductive period.
I am thankful to FOGSI for allowing me to publish this book during 48th AICOG at Aurangabad.7
The efforts shall be considered worthwhile if policy makers and program managers are accepting this book for better RCH program at rural India.
