It is a privilege to be requested to write a foreword to this book Infertility Practical Insights in Management. My eligibility to this nice task is that I had witnessed and practiced gynecology of fifty years ago. The problem of ‘Infertility’ was solely in the hands of gynecologists who treated infertility empirically with vitamins, hematinics, dilatation and curettage. Male infertility was declared hopeless for any form of treatment at that time. The scenario has changed, yes changed suddenly during the past twenty-five years and more so during the past fifteen years and the subject of ‘Infertility’ is now an exclusive specialty in gynecology. Insight in the information about ovum, ovulation, spermatozoa, fertilization and implantation and hormones has widened and deepened and is continuing to improve. Male infertility is no longer a hopeless situation. In this book five chapters out of ten are about male infertility. The authors of all the topics are renowned experts in the management of infertility. All the practical problems encountered in the management of infertility are covered in this book. Dr Jayakrishnan who edited this book has vast experience and knowledge in the problems of infertility. This book is a timely gift to all those are in search of practical and precise information in the subject of male and female infertility.

On the occasion of 49th AICOG to be held in Cochin in 2006. It gives me great sense of satisfaction and pride in editing my first book on infertility after 25 years in field of Obstetrics and Gynecology.
Our understanding of the problems in infertility has expanded rapidly with the rapidly growing field of reproductive medicine and science. Similarly, new innovations in assisted reproductive technologies have opened up numerous options for treatment of infertility and improved our understanding of the processes in gamete interaction and implantation. The increasing focus on evidence-based practice has led to critical review of existing evidence in the area of therapy, diagnostic assessment, causation and prognosis and has provided the framework for conducting clinical studies and therapeutic trails that have high validity.
The major objective has been to provide the most current scientific information. To accomplish this objective we have brought a group of recognized authorities, by being at the forefront of research in their fields in reproductive medicine, to share their knowledge with the readers.
I wish to thank all the contributing authors for agreeing to submit their work and for their quality of their manuscripts which has allowed us to produce this textbook. It would serve as a very useful resource to professionals dedicated to improving and understanding the problems associated in management of infertile couple.
