Refractive surgery has evolved significantly in the last two decades. We have seen a shift from incisional refractive surgery to laser-based procedures. Lens-based procedures have also come in a big way and are complementing laser-based surgery. Newer concepts and procedures are constantly being developed with a view to widen the scope of refractive surgery. As a result, a whole range of procedures exists now. We can tailor the treatment to the patient's requirement and provide optimum results.
Amongst an ever-increasing number of refractive procedures, the one procedure that has contributed most in making refractive surgery a widely accepted procedure is LASIK. The obvious benefits of LASIK coupled with early visual recovery, excellent and predictable results, minimal discomfort postoperatively, lesser regression and no haze have contributed to its popularity. It is still the most widely performed refractive procedure in the world.
Advanced Surface Ablations like PRK, LASEK and Epi-LASIK have developed to enlarge the scope of laser refractive surgery. Improvements in providing a better quality of vision have also taken place in the form of wavefront-guided LASIK and Aspheric LASIK.
Another recent exciting development has been the Femtosecond Laser, used for creating the LASIK flap. This makes LASIK a truly “all laser procedure” or “bladeless LASIK”. This would lead to a reduction in potential flap related procedures. Solid state lasers are already here. Depending on the commercial viability, they may have a significant role in the future.
Gone are the days when mastering a single technique like Radial Keratotomy or LASIK would see us through our refractive surgery practice. The refractive surgeon today has to be well versed in all the 9procedures and design the treatment to suit the patient's requirement. This may occasionally mean employing two or more procedures. The bottom-line today is to provide good quality vision in a safe and effective manner.
I have known Dr Shashi Kapoor for a long time now. He is a very dynamic person, constantly brimming with ideas and has a lot of foresight. He was amongst the first to put up an Excimer laser. His flair for writing is well known. His numerous earlier books have been very well received and some constitute essential reading at the under-graduate and postgraduate level.
I wish this book all the success and am confident that it will be an asset to practicing ophthalmologists.

Since its inception, LASIK has captured the imaginations of both the ophthalmologist and the patient. The rapid visual results, and the much reduced pain compared with that of PRK are attractive to the patient, and its relative ease of mastering the technique makes it popular for the ophthalmologist. Over the past two decades, techniques and technology have rapidly evolved and the advent of the intrastromal ablations makes LASIK an “all-laser” procedure an option, further enhancing the ‘wow’ factor.
With these developments, it is no longer possible for the general ophthalmologist to ignore its impact on his or her practice since the information explosion on the Internet has now allowed most patients to read up about the procedure. For arming oneself with knowledge about LASIK and its results and also to keep abreast of the recent trends and advances with this surgery, it is important for a single source of information and I believe that this book provides this.
Written in an easy to understand format by stalwarts in the field both from India and other countries, the systematic format allows anyone interested in entering this field to obtain an overview of the procedure. The well compiled chapters provide information about the basics of LASIK, the equipment used, and the newer modifications of the procedure. The book fulfills a need for a comprehensive treatise on LASIK and I wish the book and the editors all success.
