7Foreword
From the desk of Prof. Malcolm Harris presently at Postgraduate Medical Institute, Warwick University, London. E-mail address: malcolm.harris@warwick.ac.uk dated 5th February, 2008.
I have known Professor Pradip Ghosh since 1990 when he visited my department at the Eastman Dental Institute and University College Hospital as a Visiting Registrar. He was at that time Associate Professor in Calcutta. He has since visited London as a friend and surgeon. His book especially the Synopsis of Oral and Maxillofacial Surgery is an excellent comprehensive work. Pradip's recent book Step by Step Anaesthesia in Dentistry—Local and General is to be published soon. Again it is a comprehensive, concise and lucid work. I believe students, teachers and general practitioners will appreciate this book.

9Preface
To achieve painless extraction, minor and major surgery, the need of anaesthesia including local and general is absolutely ma n d at or y.
The sound anatomical knowledge and technical know how of local anaesthesia and exposure of general anaesthesia and its procedures, including mechanism of action, indications and contraindications, uses and standard modalities are essential to achieve experience and success in routine dental practice.
In mid 80's in order to fulfil my intention and desire to work in local anaesthesia, I registered in oral surgery PhD programme at Calcutta University under the guidance of emirates and legend Prof. Dinamonee Banerjee. Unfortunately, due to sudden demise of Prof. Banerjee, my work was totally closed.
At that time I communicated with Dr George Allen Edward GowGate in Australia. Fortunately, Dr GowGate sent his original thesis to the undersigned along with a few slides for my work. During that period I had also collected materials for this programme. I am personally grateful to Dr GowGate in this regard.
Early and prior to 2000, while conducting examination programme, I had noticed during moderation and paper setting that in the examination schedule syllabus 20 marks were allotted for LA and GA in Oral Surgery. 10
A few years back it was noted that some of the textbooks of oral surgery covered LA and GA or GA alone. The separate local anaesthesia books are available but there is no such book, which covers LA and an exposure of general anaesthesia, which is absolutely need based for undergraduate teaching. Step by Step Anaesthesia in Dentistry—Local and General is my humble presentation on above mentioned perspective.
In this connection I would like to mention some university questions for BDS examination on oral surgery including local and general anaesthesia. Some sample questions are as follows:
There are so many questions, which provoked me to write on that perspective.
So I have tried to explain the subject of general anaesthesia not in detail, but the term exposure would be more suitable. I personally feel the exposure of general anaesthesia is an appropriate and absolute term.
I have to mention the protocol of General Dental Council of UK, recommended by Prof. David Poswillo for use of general anaesthesia by the dental surgeon. The above mentioned protocol or guideline would provide protection to the dental surgeon of any country.
Regarding certain clarification of old technique, and use of cartridges, I am grateful to Prof. Kali HC Kapadia, Prof. PV Janardhanan, Prof. Krishnamurthy Bonanthya, Prof. Bhaskar Rao and Dr. Barin Roy.11
I cannot forget to mention the help of my teacher in anaesthesia late, Dr. Sankarananda Talukder for preparing this book. When I had discussed planning for writing this book in the month of August 2006 at London, Prof. Malcolm Harris had asked me specifically to write about sedation anaesthesia.
I am grateful to Prof. Harris to allow and arrange my library work at Eastman and White Chappel Hospital at London.
I am grateful to Major General Rtd. Prof. PN Awasthi for appraising and organizing the contents and synopsis of the present book.
I was a student in past, presently I feel that I am still a student, but designation has changed (teacher) due to change of time. My intention is to see the subject from the student's point of view, their perspectives, problems in study and how to form a solution.
The Stallwart of our fraternity may think, what is the use of writing above comments? I still feel that I am a student, my thinking and thoughts are for the students and students only. If I satisfy the students of their queries regarding the study and knowledge, my success would be achieved.
I personally believe there maybe so many mistakes and omissions, that many alterations and corrections may be necessary. In spite of that I am presenting this book to the learned readers for inviting their constructive criticism.
I am grateful and convey my thanks to Dr Kamala Prasad Singh, Chairman and DG, Sarjug Dental College, Darbhanga.
Pradip K Ghosh12
