fm5Contributors fm9Foreword
It is a privilege to write the Foreword to the second edition of the book Principles and Practice of Pediatric Nephrology, to which many eminent pediatric nephrologists across the country have contributed and has been edited by Dr Vijayakumar M and Dr Nammalwar BR, Pediatric Nephrologists, Mehta Children's Hospital, Chennai, Tamil Nadu, India.
The book has been structured into 14 sections, which have been subdivided into chapters that cover all facets of pediatric nephrology in totality, from basic nephrology to chronic kidney disease. Moreover, the thoughtful inclusion of topics of Pediatric Urology, Diagnostic Radiology and Renal Histopathology, makes it an all-inclusive blanket reference for pediatric nephrology.
It not only comprehensively addresses the need of a reference textbook for students pursuing to acquire skills in pediatric nephrology, but also serves as a ready reference for both pediatricians and adult nephrologists, while managing pediatric patients in their busy clinical practice. The highlight of the book lies in the simplicity of its approach, clarity of thought on the subject and the lucidity of the illustrations.
The rational approach to the clinical problem, suggested at the end of most of the chapters is of immense help to the practising nephrologists, and this book is a “must” in the shelf of all pediatricians, as well as adult nephrologists.

fm10Preface to the Second Editionfm11fm12
The origin of this book Principles and Practice of Pediatric Nephrology began in a small way. The teaching materials contributed by the Faculty of the Annual Conferences at Chennai of the then Indian Pediatric Nephrology Group were xeroxed, bound and distributed to the delegates and pediatric postgraduates as a ‘Textbook’. With successive conferences at Chennai, the contents and the volume of the book increased but the materials in the book was outdated. In 2004, a multi-authored new book with a fresh approach and materials was published. The Editors coined it as the Third Edition but the Publishers rightly designated it as the First Edition. Now we have the Second Edition. In this Edition, we have expanded the subjects to a wider spectrum, which includes ‘Genetics of some renal disease, molecular pathogenesis of inherited renal disease, and pediatric urology’ as applicable in clinical practice for pediatricians and pediatric nephrologists. Further value has been added with inclusion of Neonatal Nephrology, a subspecialty on the horizon, preventive nephrology, renal histology, vitamin D, and the kidney and acute kidney injury in Pediatric Intensive Care Unit (PICU) environment. Since the last Edition, there has been a gigantic surge in the number of young and academically brilliant pediatric nephrologists in India. Hence, we have extensively drawn their potentials as the resource for this book. The goal of the Editors is to provide a complete textbook of pediatric nephrology for pediatric postgraduates, a reference book for pediatricians in practice and an introductory book for internal medicine postgraduates in nephrology. If this book has fulfilled this object, it is entirely due to contributions from the authors who have put in their sincere efforts. The shortcomings are the Editors’ contributions. We request our readers, postgraduates, teachers and colleagues for their critical review, suggestions and comments to improve in the next edition.

fm13Preface to the First Editionfm14
The third edition of Principles and Practice of Pediatric Nephrology is being published after 10 years. The second edition was published in 1993. Since then, there has been constant request from the postgraduates and colleagues for the next edition. The outcome of this encouragement and support is the printing of this third edition. We have attempted to make this textbook as comprehensive as possible and at the same time to keep the presentation in simple language. Newer chapters such as inherited diseases of the kidney, genetic basis of renal diseases, renal replacement therapy and newer investigation modalities have been included in this book.
Our friends Dr N Somu, Dr L Subramanyam, Dr A Balachandran, Dr K Nedunchelian, Dr NC Gowrishankar, Dr S Lakshmi and Dr So Shivbalan, have contributed extensively in every respect in the production of this book as in the earlier edition.
We are thankful to our colleagues and senior pediatric nephrologist, for their contribution to this book. We cannot forget our friends and colleagues from India and abroad, who have contributed their experience to the book. If this book is good and useful, it is because of contributions from these Authors.
We are grateful to Dr Radhakrishna Baliga, the center pin, for the success in obtaining contributions from senior and respected faculty from abroad. We are fortunate to have the unstinted support of our former colleagues Dr Vivek Narendran, Dr Aravind Kasaragode, for their own contribution and also from their colleagues.
We are thankful to Dr HPS Sachdev, our friend and guide as well as a senior pediatrician of India, for his ‘Foreword’ to this book.
We are sincerely thankful and grateful to our friends, Mr G Suresh and Mr S Rajkumar, for their sincere and unconditional secretarial assistance.
We are thankful to our wives, children and our family members, for their patience and mental support to bring out this book.
We are grateful to M/s Jaypee Brothers Medical Publishers (P) Ltd, New Delhi, India, for their willingness to support our cause by accepting to print and publish this book, Principles and Practice of Pediatric Nephrology.
We request our readers, the postgraduates, teachers and colleagues, for their critical review, suggestions and comments to improve in the next edition.

fm15Acknowledgments
Ingratitude is the most heinous crime a human can ever commit. We are grateful to Editors of many International and National Textbooks of Nephrology, whose work formed the basis in formulating this book. We are thankful to postgraduate students in pediatric medicine and our pediatric colleagues, for their constant encouragement and impressing on us, the need for a second edition. We remember and continue to be grateful for the services of Dr Radhakrisna Baliga in obtaining the contibutions for the first Edition of this book from senior and respected faculty from abroad. The first Edition was fortunate to have the unstinted support of Dr Vivek Narendran, Dr Aravind Kasaragode, Dr Suhas Kallapur, Dr Shoba Sahney, Dr Ari M Simckes and Dr Bradley A Wardy. Their participation had enhanced the academic merit of that Edition. The spontaneous support and willingness to contribute at a short notice by our pediatric colleagues for this Edition has humbled us. We are grateful to each one of them. An individual note should be made of Dr Prajanya and Dr Tarak Srivastava, for their contribution on ‘Genetic diseases of the kidney’ and ‘Vitamin D and the kidney’. We thank Dr Gokulnath, President of the Indian Society of Nephrology, Southern Chapter for his ‘Foreword’, a matter of pride for us. Our colleagues in the Pediatric Nephrology Department, Dr N Prahlad, Dr E Sudha, Dr Shweta Priyadarshini and Dr G Sangeetha, gave their unstinted effort and time in proofreading and correction of the manuscripts, which was beyond call of their duty. We are grateful and thankful to Dr Susan Uthup, our Pediatric Nephrology colleague and Dr G Nandhini our Pediatric Surgery colleague, for their constant support in the preparation of the text and also in the proofreading. We want to place on record the positive approach and support of Dr Amish Udani, our Pediatric Nephrology colleague from Mumbai in the preparation of the text as well as in proofreading. We thank Dr S Thangavelu and Dr NC Gowrishankar, our Pediatric colleagues, for their excellent support in the preparation of this book. We sincerely thank our companion-in-work, Mr G Suresh, for his secretarial assistance in the preparation of the book from its conception to delivery. We are thankful to our wives, children and family members who bore with silence our negligence of our responsibilities to them.
Last but not least, if a beautiful book is in your hands, it is because of Shri Jitendar P Vij (Group Chairman), Mr Ankit Vij (Managing Director) and Mr Tarun Duneja (Director-Publishing) of M/s Jaypee Brothers Medical Publishers (P) Ltd, New Delhi, India. We also compliment Ms Chetna Vohra, Mr Jayanandan, Ms Sajini and others in Jaypee Brothers Medical Publishers (P) Ltd (Chennai and Bengaluru Branches), for their patience and willingness to throw light for us in the dark path of printing and publishing.
We request our readers, postgraduates, teachers and colleagues, for their review, suggestions and comments, so we learn how to improve and make our future editions better. Our efforts will be rewarded if one more pediatrician is drawn into the arena of Pediatric Nephrology.
