5Dedicated to
My father, Stuart Gopen, and my mother, Marilyn Gopen, who have stood by me in life throughout all of its challenges. I would also like to dedicate this textbook to Dr. Gerald Healy, a true giant in the field of otolaryngology, whose teachings and support have been indispensible toward the development of my career. Finally, I would like to dedicate this textbook to the medical students and residents that I have worked with throughout the years. Their continued enthusiasm served as the main driving force for this textbook.

9Foreword
It is with great pleasure that I have been asked to contribute the foreword to this new textbook designed to meet the needs of junior residents, students, physicians’ assistants, nurse practitioners and other personnel entering into the study of Otology. This book has been nearly entirely written by the principal author and editor, Dr. Quinton Gopen, with whom I had the privilege of sharing a practice in his early post-training years and it has been a joy to see him develop as an increasingly accomplished academic neurotologist. He is a wonderful teacher and mentor, with the ability to craft his careful instruction into effective writing that will make this text a particularly valuable resource as an introduction to the field of Otology.
The textbook thoroughly presents the normal anatomy and discussions of the disease processes of the various regions of the ear and temporal bone. It pays particular attention to the significant differences and considerations between adult and pediatric patients and their pathology. Systemic diseases that affect the ear and temporal bone are presented in a separate chapter. Dr. Gopen has some particular expertise in this area having spent some time in postgraduate study working in the temporal bone pathology laboratory at Massachusetts Eye and Ear Infirmary under the late Dr. Saumil Merchant, who has been another wonderful mentor. The book concludes with well detailed and illustrated step-by-step presentations of the principal otological operations. The readers will surely enjoy the thorough and thoughtful teaching that Dr. Gopen has carefully put into this new textbook, Fundamental Otology: Pediatric and Adult Practice.

11Preface
The goal of this textbook is to provide a substantive coverage of the fundamental aspects of otology. Although it is always difficult to determine the most appropriate scope of any textbook, a concerted effort to include pediatric otology in this textbook was considered a priority. The first 9 chapters include both pediatric and adult otology and are organized based on anatomic sub-sites. Each chapter includes a brief embryologic review, a more comprehensive anatomic review, and a discussion of pathologic disease processes. Although skull base surgery is certainly a complicated and expansive field, some of the basic principles and fundamental surgical techniques regarding skull base surgery are an important part of otology and are consequently included here in Chapter 9. Chapter 10 is a thorough review of common and uncommon systemic diseases focusing on their otologic manifestations. The subsequent chapter on radiology focuses on normal anatomy as seen on CT scans and MR images. Finally, Chapter 12, Surgical Procedures, is an overview of the common surgical procedures performed in the practice of otology as well as some of the essential skull base procedures.
