I am very glad that The Uterus Manual, a book only about uterus is well-received by the scientific community. So, it is my great pleasure and privilege to present a second book only about another reproductive organ, The Fallopian Tubes. Today, there are many books in obstetrics and gynecology, of good quality. But to find a single organ in a single book is quite rare. Hence, this endeavor has always fascinated and intrigued me from the very beginning of my career. It is only an organ of passage but what a passage !!!, this 10 cm length of a tunnel through which the humanity exists, the humanity continues to eternity. This is the passage, where the ovum meets the boyfriend or girlfriend unites, forms an identity and travels, the first step undertaken in its journey till his or her death and demise. What a fantastic road!!!.
Our branch has changed very rapidly with recent advances and techniques. Many new investigative procedures and management protocols have came into existence which were not before, the practice is becoming more evidence-based and the guidelines are rapidly changing.
To get and beget is the basic human driving force. Hence, infertility is a problem of immense magnitude and out of the many causes of infertility, tubal factor infertility is a very delicate issue. Once we diagnose a damaged tube, we give the couple—a syndrome ‘the empty nest syndrome’ and a life of hopelessness.
The function of the fallopian tube is ovum pick-up, transportation of spermatozoa, fertilization, ovum and embryo transport, but still there are many gray areas. So many things about the tubes, we do not know. After more than half-century of clinical interest in female infertility, we are unable to assess accurately a single physiological function of the fallopian tube. Our clinical armamentarium is limited to appraisals of the patency of the tube to gas or liquid, and to examination of the external appearance and texture of the tube with regard to the ovum and embryo, we know little about the mechanisms of ovum transport, the importance of synchrony between the ovary and the tubes. With respect to the oviductal factor, we know very little about the critical length of residual oviduct required for function or which, if any, regions are critical to function. To answer few of the questions, the book focuses on the state-of-the-art aspects of fallopian tubes including reviews of normal, abnormal, physiologic, biochemical and molecular aspects of the organ. It explains the basic mechanism of inflammatory cell activities, to molecular biology to pathophysiology of different diseases affecting the organ, the therapeutic modalities adopted from time to time as science progresses, from ways of preventing the diseases affecting the organ and the optimum management.
The task of writing the book to ensure that it is up-to-date at publication, has been a taxing one, not for us only, but for our families too. But for me, it has been an educational experience during which time me and my contributors had an unusual opportunity of examining our clinical practices in the light of the published evidence.
I hope that the book will be helpful to postgraduate students, practicing obstetricians and gynecologists, who deal with the many patients who suffer from tubal diseases. My endeavor will be successful, if they are benefited from it. We hope the readers will find the book as helpful as we have found during writing while at the same time being spared the pain. I wish you all a happy and thorough reading.
