9Foreword
The vital relationship between women's reproductive functions and mental health has been recognized for over 2,000 years when hysteria was attributed to wandering of the womb. Fortunately, women no longer undergo hysterectomies to cure mental illness and the relationship between obstetricians/gynecologists and psychiatrists has developed in many cultures into rich and meaningful clinical, educational and research endeavors that benefit women's mental and physical health.
Menstruation, fertility, contraception, sexuality, pregnancy, births, lactation, abortion, and menopause are all laden with deep physical and emotional significance. Pathologies, such as infertility, sexual dysfunction, sexually transmitted diseases including HIV, fetal abnormality or loss, chronic pelvic pain, breast disease, and gynecologic cancers, are psychologically distressing to even the most resilient women. Relationship issues between the women and her partner, her parents, in-laws, friends and her healthcare professionals may also be areas of concern to both the psychiatrists and obstetricians/gynecologists. It is clearly helpful for both specialties to have a working knowledge of the other's domains to improve collaborations. This initiative, one of the first books in India on this topic, makes a vital contribution by describing some common psychiatric conditions and treatments that may be useful to the specialist in reproductive medicine and gynecology.
The complementary roles of obstetricians/gynecologists and psychiatrists are particularly salient in the context where gender equity and rights still need much improvement. Fetal sex selection, male child preference in birth right, nutrition and education, child brides, dowry disputes, intimate partner violence, sexual assault, and high suicide rates in women all beg for intersectoral and interprofessional collaboration for prevention and optimal treatment.
Dr Sunanda Kulkarni (Professor, Department of Obstetrics and Gynecology, AIMS, BG Nagar and Formerly at Bangalore Medical College and Research Center, Bengaluru) and Dr M Kishor (Assistant Professor of Psychiatry, JSS Medical College Hospital, Mysuru and formerly at AIMS, BG Nagar), and their contributing authors are to be commended on this first edition of Essentials of Psychiatry for OBG Practitioners which should be of great interest to both specialties and their students. Clinician's in obstetrics and gynecology have unique role, unlike any other branch of medicine, they take care of two lives at same time. Obstetrics and gynecology practitioners come across thousands of women in their career, who are thankful for their service at the most important moments of their lives. The darker side of womanhood is that many who seek consultation may suffer silently considering mental agony as ‘normal’ during many phases such as in puberty, pregnancy, postpartum, postmenopausal period, etc. This increases the morbidity and mortality. Clinicians are also misled by the myth that anxiety, depression, and suspicions are part of normal behavior, even when patients narrate them. This is also compounded by clinician's lack of adequate exposure to psychiatry in their undergraduation as it is optional subject. A clinician who is aware of common psychiatric disorders, such as depression, can play a pivotal role in reducing the morbidity and mortality because depression affects nearly 10–20% of women in their lifetime.
This book is an effort to create awareness and knowledge about psychiatry in comprehensive manner relevant for obstetrics and gynecology practitioners. It helps obstetrics and gynecology practitioners to identify the psychiatric symptoms, their cause and management issues. Care has also been taken to present the contents in simple language. Liaison of psychiatry and obstetrics and gynecology specialties can ensure wellbeing of mother, healthy baby and a thankful family, in majority of women who seek help.
As editors, it was challenging task to get more than 35 well-known consultants in the fields of psychiatry and obstetrics and gynecology to author chapters and publish this book. We are aware of challenges ahead. We hope readers will send us critical feedback to improve in subsequent editions.
