Neck Ultrasonography (US) has become an extremely useful and indispensable “physical examination” tool in the clinical evaluation of patients with thyroid and parathyroid disorders. Clinicians from different disciplines frequently use this highly accurate and dynamic imaging procedure to verify their physical findings, discover non-palpable lesions, estimate lesion size and risk of malignancy, in addition to defining the characteristics and extent of lymph node involvement, to mention only a few of so many established clinical uses of neck ultrasonography. Not only that US is used in the initial evaluation of thyroid pathology but it has taken a primary role in the follow up of patients who were operated for thyroid cancer. The accuracy, affordability and lack of radiation have made this radiological test a new clinical skill for the endocrinologists and endocrine surgeons. Real time US examination performed in the clinic coupled with the clinical and biochemical data that are available to the clinician at the time of patient’s evaluation result in a more evidence-based decision making and a better management plan.
The challenge for the clinician who is interested in using neck ultrasonography is to master the technique and wear the gown of the radiologist in the clinic. This needs a significant amount of training and practice. Clinicians, who are interested in acquiring these skills need to attend courses, study basic textbooks about US and practice as much as possible. One of the most helpful tools to enhance the ability of the clinicians to learn how to interpret images of US is to view real images and correlate them with patients’ clinical data and other studies using other imaging modalities. The “Pictorial Atlas Of Thyroid Imaging” provides an excellent source for learning thyroid US through these methods. Drs. Mustafa and Bhatt have put together an outstanding resource of carefully chosen ultrasound images that cover a wide range of thyroid and parathyroid disorders. They wisely supplemented this with other radiological imaging modalities to allow the reader to understand the pathology and build a full picture of the structural changes in the thyroid and parathyroid glands. The cases included in the Atlas are the “bread and butter” of daily practice of thyroidology and are concisely well described and frequently supported by other radiological studies. This would enhance the knowledge and skills of those who are interested in learning the basics of interpretation of thyroid and parathyroid US images and deepen the skills of those who have some experience with this radiological examination technique.
10The Atlas is certainly a welcomed addition to the relatively small number of books and monographs that are specifically directed to the large number of clinical endocrinologists and endocrine surgeons who aspire to learn this art of thyroid evaluation skills. 11Introduction to the First Edition
Thyroid structural pathology is increasingly encountered in clinical practice. The prevalence largely depends on the method of screening and the population studied. The variety of imaging in modern medicine, not only has influenced the direction of diagnosis and management of thyroid disorders, but it has posed major challenges to the clinician. The mission of the first edition of this “Pictorial Atlas Of Thyroid Imaging” is to present complex imagery in an easy-to-follow way, comprehensible by those who may not be fully trained in radiology. Almost every case was illustrated in multiple views, meticulously chosen, to demonstrate the pathology of interest. In many instances, various imaging modalities were presented in parallel for the same case. The current Atlas format was substantiated by a clear, systematic narrative description of each image. This makes it an ideal tool to aid in learning fundamental radiology reporting skills by example.
Although the main bulk of the images are Ultrasonography US), following the AACE reporting guidelines with all measurements are longitudinal x transverse x antero-posterior cm respectively, we have attempted to combine a wide spectrum of thyroid imaging technology, like direct photography, CT, MRI, PET scans, nuclear medicine studies, and other molecular-based images, to enhance the understanding of various representations of a given thyroid pathology and other related structures in the neck. The great contribution of experts in the feld with unique and fascinating images collected over many years of experience has enriched the compilation and diversity of the presented material.
We believe that this“Pictorial Atlas Of Thyroid Imaging”offers a comprehensive, yet quick, and easy visual reference, meant for endocrinologists, radiologists, and thyroid surgeons who would like to configure common thyroid pathologies encountered in daily clinical practice.
