This book is dedicated to the memory of my dear father
Dr Harjit Singh
Former Professor and Head, Department of Medicine
Government Medical College, Patiala
(10-07-1926 to 06-03-2018)
Who was an astute clinician and a passionate teacher whom we lost this year. We always used to see him reading latest journals even after his retirement. He graduated from Government Medical College, Amritsar, in the year 1953 as the best graduate and the best athlete of the college.
The book is also dedicated to all our teachers and physicians of his era who did not have access to search engines, databases, meta-analysis, and practice guidelines that we have today, who all depended solely on their wisdom and interpretation of original studies for evidence based practice of medicine.
Of course, they depended more heavily on teaching by their seniors and mentors and not on the electronic media that we tend to depend on now. The book carries expert commentary by senior clinicians on original articles published in 2017.

Rapid new advances and awareness of existing phenomenon are happening in the present day era of evidence based medicine. It is good to look at the new updated information that is available regarding clinical presentation and management of diseases. Effective, safe, and scientific patient care requires update of knowledge, access to information and ability to interpret and then use this information for better outcomes. Medicine has righty been recognized as a science and an art. The scientific aspect is well depicted in the abstracts from leading journals which have been taken out in this book that impart meaningful information. Information is easy to get today but its appropriate application determines and decides a clinician to be good or average. The art regarding interpretation of this data and how to use it for patient care is there in the commentary by the experts who have narrated in a simplified manner the clinical application of this data.
The amount of literature in contemporary medicine is enormous and to extract 20 most suitable articles from each subspecialty that are useful for a postgraduate student and a practicing physician is itself a task that has been accomplished very appropriately. The book is going to be very useful to update regarding the new developments and to see the direction in which different subspecialties are moving for better patient care. A physician who takes care of patients with diseases of all subspecialties and has the distinction of looking after a person in totality rather than just one system needs to be well versed with the new developments in each subspecialty. In that sense, this year book will serve a great purpose as it tells you the current thinking of the specialists. We wish the readers will be able to apply the knowledge gained here for better evidence-based care of patients since we live in an era which needs transparency, openness, cooperative wisdom, and auditing of what we all do. Please do not forget that all this knowledge has to be used with compassion, empathy, concern, and kindness for the patients. We congratulate the authors and the publisher for filing in this gap since no year book of medicine exists at present for the benefit of the readers.
PREFACE
“A physician is obligated to consider more than a diseased organ, more even than the whole man—he must view the man in his world”
–Harvey Cushing
The Year Book of Medicine 2018 is an effort by experts from across the country to look at the new evidence that has been published in the medical literature in the year 2017. We have divided the book into the following sections: (1) Cardiology, (2) Chest and Critical Care, (3) Diabetes and Metabolic Disorders, (4) Gastroenterology/Hepatology, (5) Infectious Diseases, HIV, and TB, (6) Miscellaneous (Geriatrics, Genetics, and Pregnancy Related), (7) Nephrology and Hypertension, (8) Neurology, and (9) Rheumatology and Immunology. In each section around 20 articles have been chosen from three kind of journals: (i) high impact factor journals, (ii) speciality journals, and (iii) journals with publications from India. Mostly original studies have been taken. At places review articles which make useful reading have also been included.
We have not taken any guidelines since the purpose was to critically evaluate the impact of various new studies that have been published in the year 2017. The aim was to provide simplified commentary on the articles for the benefit of postgraduate students and practicing physicians. The subject experts of each section have independently prepared their own section assisted by two of their associates. The abstracts have been published as they appear in the journals where we could get the paid permission for the same. The few journals where we could not get the permission, the abstract has been rewritten keeping in mind that the main message is retained. These abstracts are mentioned as “redrafted abstracts.”
I am personally thankful to Mr Jitendar P Vij (Group Chairman) whose idea it is to bring out this year book since none exists at present. We have tried to fulfill his desire to the best of our abilities. I would thank the management, my colleagues, and the library staff of Dayanand Medical College and Hospital. The staff of Jaypee Brothers Medical Publishers (P) Ltd. who need special thanks includes Mr PN Venkatraman, Dr Neeraj Choudhary, Ms Sanjana, and Ms Neha. It has been a pleasure to work with the section in-charges who are the best in the field in the country. Muskan Sharma has been the architect of this book and has spent many hours in drafting, coordinating, and editing. But for her efforts this book would not have been possible.
I would like to thank all section in-charges and their colleagues who have put their thoughts about these articles in a simplified manner. Let us all thank the almighty God for giving us strength to accomplish this work. We pray to God to enable us to bring the year book on a regular basis well in time.
