fm6Preface
“Many things we need can wait. The child cannot. Now is the time his bones are being formed, his mind developed. To him, we cannot say tomorrow, his name is today.” —Gabriela Mistral
These lines by Gabriela Mistral aptly sum-up the importance of early diagnosis and treatment of growth and endocrine disorders in children. This is a branch of pediatrics in which early diagnosis and prompt institution of appropriate therapy has far-reaching consequences on eventual intelligence, proper growth and development, well-being during childhood and adjustment to adulthood.
This book is written with the pediatricians and postgraduate students of pediatrics in mind. It is the pediatricians on whom the onus of recognition of these disorders falls as it is they who see the child on a regular basis and therefore must identify these conditions early. It is a widely held misconception that these disorders are rare and difficult to treat. With the size of our child population, these disorders are not uncommon, and investigative facilities and treatment are now easily available. Proper choice of investigations can go a long way in reducing costs. Proper management can have effects which can transform the life of a child. I have tried through this book, to make these disorders easy to recognize and manage so that these children may be benefited.
I have been fortunate to have seen the speciality of pediatric endocrinology develop in this country right from its birth to what is now a full-fledged speciality. I had the privilege of working at the Bai Jerbai Wadia Hospital for Children in Mumbai, Maharashtra, India, for several years with Dr Meena Desai, to whom goes the credit of starting this speciality in the country. Starting from just a follow-up clinic for hypothyroid children, the scope of our work gradually widened to include other pediatric endocrine disorders and the clinic became the first pediatric endocrine service in the country. Investigative facilities were limited and available only at select centers at the time. Interest in the speciality gradually spread and as many hormone assays are now freely available commercially, it has now become a well-developed subspeciality of pediatrics with centers in many cities of the country.
It was at the suggestion of Dr Nitin Kadam, Trustee of MGM Medical College, that I decided to write this book and I thank him for his encouragement and constant support.
I cannot end without expressing the gratitude I have in my heart towards the Bai Jerbai Wadia Hospital for Children where I worked for more than 25 years, which included the formative years of my career. The experience I gained there has proved to be invaluable.
