Dear Friends,
It is a pleasure to see Critical Care Update 2021 finally taking shape and ready for release despite busy and taxing year for medical professionals in general and intensivists. Credit goes to authors, editors, and publishers. The selection of topics for the book have been well thought and are of contemporary relevance such as ARDS phenotype, long-term consequence of ARDS, HFNC or self-inflicted lung injury (SILI). Same is true of section on COVID or organization/Ethics/ICU research/quality. Due importance has been given to use of ultrasound in critical care units and should encourage more units to use this powerful tool in diagnosis, research and to ensure safe interventions. Update also touch upon end-of-life care and organ transplantation as well as on live issue of fungal infections in critically sick including antifungal stewarding. Monitoring also has got due place be it pulse oximetry nuances, fluid resuscitation targets or of patients on CRRT.
Another remarkable feature of the book has been the faith put in comparatively younger intensivists, who are leading light in their institutions, have established credentials and are likely to be future leaders in their respective fields. It has brought freshness in the approach and inline with the thinking of ISCCM leadership to nurture talent, encourage and mentor without compromising on quality and ethos, epitome of professionalism.
I fall short of words to express my gratitude and appreciation to Dr Subhash Todi, one of the senior most intensivist of the country, for his untiring effort, wisdom, guidance and mentoring the project. Same is true for Dr Deepak Govil (President Elect, ISCCM), Dr Subhal Bhalchandra Dixit (Past President) and Dr Srinivas Samavedam (Dynamic General Secretary); trio worked tirelessly for completion of the book despite resurgence of pandemic.

Preface to the Third Edition
It gives us an immense pleasure to release the prelims of Critical Care Update 2021 during the virtual Annual Congress of our society to be held from 26th–28th February 2021. In these trying times of new normal, it is imperative that the fundamentals and standards of medical education and research continue to be maintained. The current update book is 5th in the series which have been released during ISCCM Annual Congresses since 2017. The editorial board over a series of virtual meetings have selected topics which have current relevance, clinical applicability with addition of new information on the subject. Core topics have been retained, while newer topics not covered in previous editions have been added. It is not surprising that a section on COVID-19 related topics have been added this time. The editorial board have tried to strike a balance while selecting authorship by inviting a completely new fleet of authors from Fellows of College, Societies, and Academic wing selected on their area of expertise, and at the same time, retaining experience of Senior Faculty as Section Editors. As the previous, update books are well read by postgraduates, it gives an opportunity to the newly inducted authors to establish a national presence through the book. The editorial board really appreciate authors’ and reviewers’ efforts to return the chapters at a short notice, in spite of such hard pressed for academic time in the current situation. We sincerely hope that by 2022 Annual Congress with COVID behind us. We will be able to bring you similar update with more national and international authors’ contributions.

Preface to the First Edition
Dear Friends,
New Year Greetings from the Editors of Critical Care Update 2019, the Annual Congress book of ISCCM. The latest edition of Critical Care Update will be released during the Silver Jubilee Conference of ISCCM to be held in Mumbai on 1st February 2019. Similar to previous two editions (2017, 2018) this edition comprises of 101 Chapters divided across 13 Sections with a coverage of all the major subspecialties of critical care. The ratio of chapter distribution is in keeping with the relative scientific work done in the respective field. ISCCM has allocated a substantial space to the Quality, Research and Organizational aspect keeping in mind the growing importance of these often neglected areas of critical care. Most of the chapter topics will be covered during the annual congress and will be a ready reckoner for the attendees. The topics have been carefully selected by the ISCCM National Scientific Committee and the contributors are mostly national and international faculty in the congress. Young talents in the field have been encouraged to contribute to the book and the trend will increase in future.
